Physical Therapist - Position Description

Position Scope:A Field Physical therapist is a licensed professional who provides direct and indirect patient care services under a physician’s plan of care and in accordance with the policies and procedures of the Option Care, the state’s professional standards, and regulatory entities. The Field Physical Therapist is responsible for collaboratively establishing and implementing the care plan, educating patients/caregivers and evaluating the patient’s progress toward goals.
REQUIREMENTS
QUALIFICATIONS:
Minimum Requirements:

Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in collaboration with the American Physical Therapy Association or its equivalent
Current license by the state of practice Board of Physical Therapy
Valid and current drivers license and evidence of automobile insurance
Minimum of one year of experience as a physical therapist
Flexibility and the ability to work effectively within a multidisciplinary team
Ability to communicate effectively (verbal and written) with team members, patients, care givers, physicians and referral sources.

Preferred Qualifications:
Previous home health care experience
