We are currently seeking a Social Worker (MSW) with previous hospital experience. This member will work within various specialties focusing on discharge planning and assisting with psycho
osocial needs of patients and families.


The MSW proactively identifies necessary planning for the next level of care through ongoing assessment, counseling, referral, information and guidance during hospitalization. The MSW is equipped with the skills to manage patients and families in crisis, end of life issues, ineffective coping and associated dynamics in a holistic model of care. The MSW maintains positive working relationships with internal & external partners to access appropriate services.


A Masters degree in Social Work from an accredited program is required. Computer skills are required. A minimum of two years experience in a hospital is required and prior Med Surg. Experience preferred. Arizona LMSW or LCSW is preferred.
