Bachelor's Degree requiredTo provide pharmaceutical care through service as an accountable member of the healthcare team in assigned areas of the hospital or clinic with focus directed on outcomes of medication related to cure of patient disease, elimination or reduction of symptoms, arrest or slowing of disease process and prevention of disease or symptoms. A primary foundation for all pharmaceutical care is Patient Safety, which must remain paramount in all employement activities, distributive or clinical, throughout the department of Pharmacy.Supports and supervises day to day pharmacy drug distribution systems for assigned area(s) to provide medication in a manner which adheres to Gundersen Lutheran policy standards as well as existing standards of practice for the profession.Develops and maintains a clinical practice which supports and promotes education and research, and participates in or supports student and resident experiential learning.Serves Gundersen Lutheran and its customers while understanding and supporting Gundersen Lutheran Mission and Vision statements and exhibiting Gundersen Lutheran Core Values in their daily work practices.Education/Learning Experience
Required: B.S. Degree in Pharmacy. Current License or Eligibility for licensure in the State if WI.
Desired: Pharm.D Degree, ASHP Accredited Residency.Work Experience
Required: New Graduates accepted for beginning positions and duties.
Desired: Previous Hospital, Clinic, or Ambulatory Pharmaceutical Care Experience.Skills/Knowledge
Required: Knowledge and skills required to practice as a registered pharmacist providing Pharmaceutical Care in Hospital or Ambulatory settings.
Desired: Any specialty training or experience in Anti-coagulation, Lipid Clinic, Drug information, Consult Services, or other aspect of pharmaceutical care.Licenses/Certifications
Required: State of Wisconsin Pharmacy License
Desired: ACLS, CPR, BCPS (Board Certified Pharmacotherapy Specialist) in relevant specialty.Physical Requirements/Demands of the Position
Long term standing, bending, squatting.
