HOSPICE RN

Purpose: The Hospice primary RN will provide direct patient care services to hospice patients, supportive care to the patient’s family, manage a mini-interdisciplinary team consisting of LPN’s, HHA’s and support staff to enhance the quality of hospice care and case manage all assigned patient clinical records.

Responsibility: It is the responsibility of the Hospice Primary RN to be the primary liaison between Hospice Family Care, Inc. and the patient/family to ensure the highest quality and continuity of care and assure all assigned patient clinical records meet regulatory and policy/procedure requirements.
REQUIREMENTS
Qualifications:
1. Currently a licensed Registered Nurse in the state where the hospice services are provided.
2. A minimum of one (1) year experience, either in oncology, long-term care or home health.. School internships and clinical hours may be considered as experience in individual cases.
3. Hospice Primary RN’s must have their own reliable transportation, a valid driver’s license, and proof of automobile insurance
