RN's Looking for the Opp. To WORK FROM HOME!

The work at home RN's role is to provide culturally appropriate support to outreach disease management services and Community Based RNs. Provides language and cultural translation services, and supporting coordination of care and disease management activities.
Perform community outreach activities to assist community based DM program staff in locating member, inform eligible members of Disease Management services available, and promote and build trust for those services. Activities include, but are not limited to:
1.Contacting providers of care.
2.Physically visiting member’s address. (25%)
3.Telephonic Case Management (75%)

Assist in coordinating members’ relationships with multiple service systems.
Support disease management efforts.

This may include activities such as:
Utilize the computer system to maintain confidential and accurate records of member address and telephone information, documentation of all contacts and the type of assistance provided. Disseminate information in confidential format to DM Program staff.MUST HAVE RN LICENSE IN FLA
Must reside in and share experience, language and/or culture of the community to be served.
Must have valid driver’s license and liability insurance coverage.
Bi-lingual preferred; Spanish and English
High School Diploma or GED preferred
Minimum of 2 years experience as nurse.
Must have work from home office, and high speed internet.
