RN Case Manager for Hebrew Medical Center

Non-Union, Full-Time. As the right hand to the Nurse Manager, the Internal Case Manager oversees Patient plan for care functions and serves as both teacher and clinical resource for staff on a 24 hour basis. Links together the patient, the clinical team, and the family. Ensures that care progresses within the set time frames and essential equipment and supplies are readily available and used efficiently. Oversees unit management in the absence of the Nurse Manager.

REQUIREMENTS
Registered nurse, current Connecticut license required. Masters in nursing or health related area or actively pursuit of degree preferred. Three (3) years experience in gerontology, staff education or supervisory role. Certification in Gerontological Nursing and Subacute is desirable. Computer literate.
