Outpatient Rehabilitation Clinic RN
Marianjoy Rehabilitation Hospital is a 120-bed hospital in Wheaton, Illinois, that is dedicated to the delivery of rehabilitative medicine. Founded in 1972 by the Wheaton Franciscan Sisters, the hospital is part of Marianjoy, Inc., a network of care that manages more than 400 beds within the greater Chicago area. Throughout the network, Marianjoy provides inpatient, comprehensive outpatient and subacute rehabilitation services. Marianjoy is a member of Wheaton Franciscan Services, Inc., located in Wheaton, Illinois. Sponsored by the Wheaton Franciscan Sisters, Wheaton Franciscan Services, Inc. owns and operates more than 100 health and shelter service organizations in Colorado, Illinois, Iowa and Wisconsin.The Outpatient Clinic Rehabilitation nurse interacts with the interdisciplinary team to ensure the care of the clients by providing a range of nursing activities, evaluation, prevention and wellness education groups and treatment techniques in accordance with program policies, protocols and professional standards.Specific Responsibilities:Patient Safety
A.Assesses, plans, intervenes and evaluates the client's safety status and educates the clients/family accordingly.
Communication
A.Communicates effectively with clients and their family informing them of necessary information regarding their health status and rehabilitation progress.
B.Provides clear, concise and timely documentation regarding the nursing services provided and care management areas discussed.
C.Collaborates with interdisciplinary team and appropriate medical doctors to coordinate services to optimize the client's use of available resources to implement a successful discharge plan.
D.Serves as communication link between clients, family and the interdisciplinary team to appropriate doctors regarding medical issues.
Nursing Services
A.Perform wellness and prevention education groups.
B.Develops an individualized plan of care based upon nursing diagnoses/problems with consideration of client's culture, age, and developmental level and sets appropriate goals.
C.Implements a plan of care to achieve established goals and continually evaluate clients and family response to the plan of care.
D.Provide clinical nursing services including: wound care; dressing changes; blood pressure checks; bowel and bladder program; colostomy care; glucose level checks; catheter changes and client training for self care.
E.Provide training on diet education/ meal plans.
F.Utilizes pulseoxsimeter.
G.Provide family training on diagnosis.
H.Initiate quality improvement activities and co-treatments with interdisciplinary team for client/family education.
Care Management Functions
A.Participates in team conferences to update the interdisciplinary team on current medical status and clients/families awareness of diagnosis.
B.Assist and participate in medical meetings and roundings with medical doctor.
C.Follow through and coordinate medical doctor recommendations to client's other health care providers.
D.Assist with discharge planning and referring to community resources.The approximate schedule for this position is Monday through Friday from 10:30 - 1:00pm or 11:00 am to 2:00 pm. The anticipation is that this position will work 3 hours a day, 5 days a week.
REQUIREMENTS
Education:Current Indiana RN license required.
CPR certification is required.Experience:Two years of nursing practice is required.
Two years rehabilitation nursing practice is preferred.Knowledge:Applicable nursing knowledge to provide services to rehabilitation clients and their families.
Knowledge of rehabilitation process including: Community resources, programmatic protocols, UR policies/criteria, clients /family/team goals, medications, and medical terminology.
Computer experience is required, Word and Excel software experience is preferred.Interpersonal:Excellent written and verbal communication skills.
Ability to work independently within own discipline, yet practice within an interdisciplinary framework.
Flexibility and ability to manage work in a changing environment.

Lifting:
Transferring of adult clients weighing 100-300# with varying levels of independence from maximum assistance to minimal assistance. (As needed).
Lifting of wheelchair weighing 40-50# as needed.
Lifting of oxygen tanks weighing 15# as needed.
Sedentary requirements to lift material supplies from floor to overhead and floor to mid-body on infrequent basis.Carrying:
Miscellaneous carrying of materials/equipment/supplies less than 15# variable distances within the clinical. Carrying tasks performed frequently.Push/Pull:
Assisting clients in wheelchairs. (As needed)
Positioning clients. (As needed)
Reaching: Sedentary requirements performed constantly at mid-body level, frequently at overhead and low levels.
Stooping: Sedentary capacities performed frequently from floor to mid-body.
Additional Physical Job Requirements:
Lifting, pushing, and pulling with client transfers.
