Nurse Case Manager

Under general supervision of the Director and/or Supervisor of Case Management, the Case Manager is responsible for achieving annual LOS goals, coordinating efficient discharge planning and assisting with capacity solutions for Florida Hospital. Works with the multidisciplinary healthcare team to develop, activate and efficiently facilitate patient treatment and discharge plans. Meets with patients/families and healthcare team to determine patient preferences for discharge needs and with providers to coordinate the discharge plan. Demonstrates expertise in thorough and timely admission and continued stay criteria and facilitates timely interventions to assure proper patient placement across the healthcare continuum. Completes Avoidable Delay forms for all identified delays in treatment, consultation, transfer and/or patient discharge. Actively participates in outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all.
All positions are full time except for one, which is per diem located at our Celebration Health campus in Celebration. Please indicate which campus you are interested (Orlando or Celebration.
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
· Demonstrates through behavior Florida Hospital’s core values of Integrity, Care, Balance, Excellence, Stewardship, and Teamwork as outlined in the organization’s Performance Excellence Program booklet.
· Supports and facilitates campus and system goals for capacity management and LOS, identification of Avoidable Delays at department approved levels, and other Case Management initiatives.
· Completes comprehensive assessment of patient situation utilizing Case Management worksheet, review of hospital computer system and medical record, collaboration with health care team and discussion with patient/LAP. Anticipates and establishes realistic discharge plan and identifies and resolves potential barriers to plan achievement.
· Communicates effectively utilizing all available internal and external methods, e.g., wireless, fax, voice mail, email. Documents all interventions in an accurate and timely fashion and in hospital approved format, including progress notes, statistics and patient/family involvement/choice in discharge plan.
· Reviews patient treatment plans to proactively identify opportunities to facilitate efficient movement through the healthcare continuum, coordinating all aspects of hospitalization and discharge planning and ensuring effective and efficient utilization of resources.
· Applies knowledge of local, state, and federal programs to expedite discharge while maintaining sound stewardship of healthcare resources.
· Incorporates age and developmental needs of the patient population into the discharge plan.
· Reviews IQ criteria, hospital computer system, and medical record to facilitate timely and appropriate LOS and discharge plan.
· Ability to establish and maintain positive and effective physician/Case Management working relationships.
· Performs other duties as assigned or directed to ensure the smooth and successful operation of the department.

EDUCATION AND EXPERIENCE REQUIRED:
· LPN
PREFERRED EDUCATION AND EXPERIENCE:
· RN with BSN
· Health care experience
LICENSURE, CERTIFICATION, OR REGISTRATION REQUIRED:
· For LPN or RN – Florida State Board of Nursing license
