Hospice of Larimer County
Join a team of compassionate, caring professionals as they provide hospice care to patients and families.
*Admissions Director Full Time Lead the admission care team, ensure prompt agency contact and assessment for patients referred for hospice care, ensure smooth transitions to assigned staff, responsible for assuring compliance with all licensing, local, state, and federal standards, guidelines, and requirements. Excellent communication skills, participate on management leadership teams. Bachelors degree, Masters preferred, minimum 3 yrs management/supervision, current CO RN license. Experience in hospice or home health preferable.
*RN: Care for home care patients. Primarily days Mon-Fri, no regular hours. BSN preferred, current CO RN license, 2 yrs general nursing, understanding of Hospice philosophy, home care and/or hospice experience preferred.
*C.N.A. Home Care -C.N.A. and CPR certification, minimum 2 yrs experience, preferably homecare and/or hospice. Primarily day shifts Mon-Fri 8-5, 32 hrs/wk, occasional weekends.
*RN Home Care - Primarily days Mon-Fri, 32 hrs/wk, BSN preferred, current CO RN license, 2 yrs general nursing, understanding of hospice philosophy, home care and/or hospice exp. pref See job descriptions at www.hlchospice.org
