Home Care Coordinator RN/LPN

As the Home Care Coordinator you will facilitate home care and hospice referrals, and educate the Community on services available through VNS.This team player:
1.Accepts physician’s orders via telephone.
2.Communicates with referral sources including hospital and ECF discharge planners, doctors’ offices, private individuals, and case managers to gain necessary information to care for individuals in the home.
3.Investigates and determines initial client benefit coverage for requested programs and services.
REQUIREMENTS
1.You must posses a valid Indiana RN or LPN licensure .
2.We prefer you have a minimum of two (2) years home care nursing experience, with some experience in Intake and benefits verification.
3.Must have solid computer skills for inputting and accessing client referral data.
4.You must have a great personality and the desire to joining a winning team!
