Position Scope: A Clinical Liaison assures proper placement of patients within the Homecare setting by gathering preadmission information, to assists in better communication of information about the patient to all disciplines, and oversee the patient's transition to home care.
REQUIREMENTS
Qualifications:
Minimum Requirements:
Graduate of an accredited school of nursing with valid license in state of practicewith 2-3 years of experience in the Homecare setting.
Must possess management/problem solving and organizational skills.
Experience in Home Infusion, Case management, discharge planning, and home care coordination.

Preferred Qualifications:
Preferred Home Health Care Experience.
Previous Infusion Experience.

TASKS AND STANDARDS FOR POSITIONS:
Interacts with area hospitals on a daily basis to conduct assessment of patients selected by the hospital for care and who have been referred to Option Care.
Communicates timely with the discharge planner/social worker to coordinate needed HomeCare services on patients that have been referred to Optioncare.
