Clinical Dietitian

Plans patient’s nutrition care program, including nutritional status assessment. Follow up and counseling to inpatients and outpatient. May be involved with care/services to patients throughout life cycle (Neonate - Geriatric). Researches available options and makes appropriate recommendations. Conduct nutrition screens to determine level of nutritional risk. Assess patients identified as being at nutritional risk and make recommendations to the physician in the medical record. Develop individualized nutrition care plans and document plan in patient’s medical record. Routinely re-evaluates care plans as indicated by the patient’s progress/change in treatment needs. Update nutrition care plans as indicated by the patient’s progress/change in treatment needs. Routinely participate in multidisciplinary care planning meetings and communicate with all members of the patient care team. Communicate on a regular basis with the nutrition services staff to ensure delivery of appropriate nutrition care to patients. Educate patients and caregivers/family on modified diets and nutrients/drug interactions. Routinely participate in departmental and interdisciplinary quality improvement activities. Serve as a nutrition care resource to physicians, staff and the community.
REQUIREMENTS
Bachelor’s degree from an accredited college in Nutrition and Dietetics. One to three years experience in clinical dietetics. 3 years experience preferred. Memberships in the State Dietetic Association, American Dietetic Association (ADA), and in community education groups appropriate for specialty area are preferred. R.D./Licensed Dietitian. Bilingual preferred.
