We are committed to responsible care in maintaining our position as the leading provider of pediatric health care services.Job Description: The Clinical Care Coordinator/Respiratory Therapist - Supervises, coordinates and ensures patient care activities within the home care setting. Participates in local office administration in a manner that maximizes cost effectiveness and, quality of care and services.1. Assume responsibility in coordinating care to assigned Respiratory patients. Assignment of patients will be made by the Location Director/Clinical Director of Nursing and responsibilities include, but are not limited to:
a) Initial physical assessment and acuity with recommendation for respiratory care on new referrals.
b) Initial set-up paperwork and explanation of respiratory services to patient's family.
c) Initial MD letter, physician plan of treatment, care plan and chart set-up.
d) Phone or in person contact with new patients.
e) Weekly update on all patients status with documentation of such in patient's chart.
f) Monthly home supervisory visits to assess patient, family, environment and follow-up documentation.
g) Monitors patient care to ensure quality and appropriateness of services.
h) Initiation and update of physician plan of treatment according to Company policy and applicable law and regulation.
i) Communication with referring health care team, including condition, goals, changes in care.
j) Third party payer nursing updates and re-authorization for continued care.
k) Coordination of ancillary services.
l) Provision of direct patient care as required and needed.
m) Liaison for clinical information with reimbursement sources.
n) Discharge planning as appropriate.
2. Interview, hire, orient and supervision of staff. Participates in employment decisions affecting respiratory staff, including termination as appropriate.
3. Participate with organization and implementation of new hire orientation and in-service education to staff.
4. Supervising and evaluating all respiratory staff for conformity to Company policies and procedures, law and regulation and professional standards.
5. Serve as a resource person to staffing coordinator, assist in staffing new cases.
6. Participates in Location call for patient care after hours.
7. Assists in marketing by projecting a positive image of the Company at all times through professional behavior, and positive interactions.
8. Emergency staff relief for new patients.
9. Participation in and support of the IOP Plan and activities.
10. REQUIREMENTS
1. CRTT, RRT required with current Colorado state license 2. One year of experience in a pediatric/neonatal specialty area. 3. Previous marketing experience a plus.
