JOB DESCRIPTIONThis position reports to the Program Manager and works closely with the Senior Case Manager. The primary role of the case manager is to provide direct service to the residents of Weinberg Village Waimanalo, primarily the adults who are termed “clients.” The case manager works with the clients from the time they enter the program until they leave on a one-on-one basis assisting them with improving their lives and giving them the tools they need to become self-sufficient. This position is full time requiring a minimum of 40 hours a week from 9:00 AM – 5:30. A flexible schedule, if needed, may be determined with the Program Manager. Some evenings and an occasional weekend day may be required.The duties of the Case Manager include:
•Conduct intakes on new clients upon entry into the program.
•Conduct assessments on all adult members of the household upon entry into the program.
•Conduct Urine Analysis on the adult clients (gender specific) upon entry to program and when needed on a random basis. Opposite gender will be done by appropriate staff member. Monitor client sobriety and conduct urine analyses as needed.
•Create and maintain files for clients.
•Create a Social Service Plan with the client within two weeks of their entry into the program.
•Schedule and conduct meetings a minimum of two times each month with each adult client to discuss aspects of their Social Service Plan.
•Maintain thorough case notes and update files on a regular daily or weekly basis.
•Monitor clients’ progress and suggest paths for improvements in areas where clients need more in-depth help.•Assist clients in obtaining permanent housing.
•Determine appropriate classes and/or life skills that are needed for client self-sufficiency.
•Be available to the client for emergency consultations and lend support when needed.
•Provide support for their children as needed.
•Assist clients in getting health care, welfare, furniture, financial assistance, and jobs.
•Conduct Career Kokua assessment with appropriate clients to assist them in getting started on a career path.
•Write and deliver Violation notices for any infractions of the rules.
•Work with clients to prevent evictions when necessary.
•Deliver eviction notices when required.
•Contact CPS when appropriate and all other paths have been tried to no avail.
•Provide support to the Program Manager and work as a team with other staff members.
•Answer phones, make copies and file as necessary.
•Represent the company to the public who come here and as required.
•Any other tasks as determined or required.
PROGRAM DESCRIPTIONWeinberg Village Waimanalo is a service-enriched clean and sober facility for homeless families with minor aged children. The families range from single mothers with one or two children to couples with several children. Most of these families are on welfare and some have issues with substance abuse. All have been either Homeless, i.e. living on the beach or in their cars; Sheltered Homeless, i.e. living in a treatment facility or domestic violence shelter; or At Risk of Homelessness, meaning they are being evicted, moving house-to-house or are living in an over-crowded home. Our job is to provide housing and life skills training and support which moves them forward into self-sufficiency and permanent housing as quickly as possible. As long as they follow all of the Program Rules, the residents are allowed to stay here up to two years until they find permanent housing.
We are all employees of Holo Loa'a Inc. who is the agency that has been awarded the contract with the state to manage this program. We have managed the program from the beginning in 1994. We are not funded or associated with the Weinberg Foundation. Clients pay program fees to live here. State and client funding are our only source of funds.SALARY AND BENEFITSThis is a full time salaried position, to be determined by the Program Manager and the budget allowed by the State funding. There is a three-month probationary period during the first three months of employment. During this time, the employee will be assessed for general knowledge, attention to detail, learning style, teamwork, attitude and overall ability to perform the job. After successfully completing the probationary period, the salary will be raised to the appropriate place and the employee will be considered permanent staff. This is an at-will position, meaning that either the employer or the employee can terminate the employment at will at any time. After four weeks of consecutive work, the employee will be eligible for Health, Vision and Dental Insurance through HMSA. After one year of consecutive work, the employee will be eligible for one week paid vacation and one week paid sick leave. Anytime taken off prior to the one year anniversary will require the permission of the Program Manager and will not be paid. REQUIREMENTS
CASE MANAGER QUALIFICATIONSThe ideal candidate for this position should have a Bachelors degree in Social Work, Psychology or may possess relevant experience related to this type of field. Possessing a Certified Substance Abuse Counseling (CSAC) certificate is a plus and would be very helpful, but is not currently a requirement. The candidate must be people-oriented with a strong desire to help families recover from homelessness. Knowledge of homeless issues and problems is very helpful, as well as an understanding of root causes and how to prevent homelessness from reoccurring. Excellent organizational and writing skills a must. Good computer skills a must. Patience, compassion and the ability to firmly, yet fairly guide the clients is a necessity.
