Supports Coordinator Supervisor - PersonLink

The Supports Coordinator Supervisor reports to a PersonLink Unit Manager and is responsible for supervising a team of supports coordinators. The primary responsibility of the Supports Coordinator Supervisor is to insure that services and supports are provided to individuals and their families in a timely, professional and comprehensive manner. This includes a hands-on approach with staff to:
* Coordinate the development and implementation of an initial Individualized Service Plan (ISP)
* Facilitate referrals and locate resources to provide supports and services according to MRS and PHMC timelines and procedures
* Monitor all services to insure that services are provided without interruptions
* Coordinate routine reviews and completion of annual ISPs according to timelines
* Provide services that are person-centered and community-based
Responsibilities:
* Recruit, orient and supervise (individual and group supervision) supports coordination staff
* Monitor, report, supervise and direct services provided by direct service staff
* Supervise staff in developing and maintaining an ongoing case load as directed by MRS and PHMC
* Monitor, train and supervise staff in all aspects of the supports coordination process
* Monitor, train and supervise staff in documenting their work in writing and electronically in HCSIS, MRS and PHMC systems for ISPs, reviews, case notes and activity logs, monitoring reports, incident management and other areas where documentation is required
* Assist in the analysis, writing and presentation of program findings
* Participate as part of a management team to plan, coordinate and implement program interventions, policies and procedures
* Facilitate and develop referral relationships with other organizations and support resources (providers, resources, advocacy)
* Facilitate and develop cooperative working relationships within the mental retardation service delivery system
* Facilitate and develop cooperative working relationships with educational, employment, training, mental health, health, and recreation service systems
* Empower individuals and families to advocate and obtain services for themselves
* Advocate for individuals and their families within and outside the mental retardation system
* Collect required data and complete all required documentation
* Respond to emergencies via an on call phone system
* Report incidents in HCSIS system
* Participate in meetings, trainings and conferences as required
* Facilitate and provide training and mentoring for staff
* Complete other duties as assigned
Performance Criteria:
* New hires:
1. Observe new SC facilitating ISPs, reviews and monitoring
2. Meet weekly with new staff for first three months and then adjust meeting time as needed
3. Complete informal 3 month review and give feedback/set learning and improvement goals with new staff
4. Review all paperwork as completed before finalized for the first three months
5. Complete 6 month probationary evaluation
6. Complete first annual (12 month) evaluation and annually thereafter
* Hold regular individual supervision meetings every 1-4 weeks depending on demonstrated skills and successful completion of the probationary period, with each staff:
1. Review 5 charts for case note frequency and quality of contacts
2. Review quality of paperwork ISPs, review and monitoring and incident reporting
3. Review frequency and quality of monitoring documentation
4. Review to do lists
5. Problem solve, mentor, redirect, help prioritize and intervene as needed
* Hold regular team meetings at least monthly
* Complete weekly and monthly compliance reports
* Attend weekly supervisor’s meetings
* Attend supervisor and training meetings
* Meet monthly, or more frequently as needed, with the Unit Manager
Salary:
Starting salaries vary and are based according to education, work experience and skills.

Experience:
A first line supervisor should have demonstrated supervisory skills and experience in supports/service coordination/case management plus a minimum of three years work experience in one or more of the following human services fields: families; individuals with physical, sensory and developmental disabilities; health, mental health or education. Skills in Spanish and other second languages, ASL, and other cultural or adaptive areas are also preferred.
Education:
A first line supervisor is preferred to have a Master’s Degree and four years of experience in an area of human services or sociology, political science, criminal justice, nursing, counseling or education or at a minimum this position requires a Bachelor’s Degree and three years of experience.
