Social Worker / Hospice Counselor

We are seeking highly-motivated social workers to join our interdisciplinary team by providing services to individuals facing the end of life, and their loved ones. As a hospice and palliative care case manager, responsibilities include providing patients and families with assessment, evaluation, counseling and casework to address social, economic, and psychological needs.
REQUIREMENTS


Candidates must have the following:A degree (MSW or BSW) from an accredited school of Social Work;At least one year of health care experience is required;Work experience must be in palliative care, community or home health hospice environments;Proven ability to work proactively in an interdisciplinary team environment is a plus; andBilingual and bicultural skills are desirable.

This is a full-time position.
