Social Worker - Cardeza Foundation

We currently have a Regular full-time (80hrs biwkly) position available in the Cardeza Foundation department. The primary function of thi position will be to provide direct social work services to patients with bleeding and thrombotic disorders in the outpatient care setting of the Cardeza Foundation Hemophilia Center, (CFHC), which is located in the Thomas Jefferson University Hospital complex.

Additional duties include:
Providing psychosocial assessment of these patients with chronic long-term illnesses, their families and their significant others.
Facilitating appropriate plans to meet these needs, which includes the provision of assistance across the continuum of care for a variety of age-specific populations ranging from pediatrics through the elderly.
Encouraging utilization of community resources to enable patient's optimal level of functioning, especially in areas of health insurance and employment issues.
Functioning as an integral member of the bleeding and thrombosis healthcare team.
REQUIREMENTS
Masters in Social Work from a School of Social Work accredited by the Council on Social Work Education or equivalent degree as determined by the Council.
4-5 years of medical social work experience.
Knowledge of the impact of acute/chronic illness on patients and families.
Knowledge of health insurance needs and interventions for people with chronic illness.
Knowledge of employment needs of people with chronic illnesses.
Excellent verbal and written communication skills.
Awareness of patient rights and confidentiality issues.
Knowledge of community and hospital based resources.
Ability to foster interdisciplinary team work.
