Social Worker

As a social work for a nonprofit community health center, they would provide direct medical social work services to patients and families of all ages, in healthcare settings and in the community, in order to meet the individual’s health and psychosocial needs in the promotion of wellness and cost-effective outcomes.

*Improve utilization of resources
REQUIREMENTS
*Education/Experience: Baccalaureate Degree in social work, psychology, sociology or other field related to social work.
*Experience in healthcare-related setting, agency or program including knowledge of medical terminology, Medicare/Medicaid programs and community resources.
*Excellent verbal and written communication skills.
*Other qualifications/certificates/license: Basic Life Support Certification, Bilingual (Spanish/English) preferred.
