Service Connector Case Manager

Position Summary :
Working as part of a team, the Caseworker assesses the social service needs of assigned families and provides direct casework assistance to families in the designated CHA developments, in order to resolve barriers to family and individual self- sufficiency.Essential Duties and Responsibilities:
·Completes the intake and family assessment for assigned families.
·Performs child development screens to assess the possible need for developmental services for children in the family.
·Develops service plan with the families within one week of the assessment completion.
·Develops a written service plan for each assigned family within required timeframes; implements client service plans; monitors service plan progress; and frequently reviews progress with supervisor.
·Reviews service plan progress with the family on a monthly basis.
·Refers adult clients to job preparedness program.
·Facilitates support and educational groups for clients, focusing on issues of daily living/life skills, child health/nutrition and home safety, accessing community resources, etc.
·Develops a working knowledge of community resources to use in serving clients and maintains productive working relationship with these resources.
·Contacts schools, other social service agencies or community organizations to coordinate client services or to make inter-agency referrals.
·Advocates for family with other service providers and social systems (school, DPA, etc,) when necessary.
·Monitors school attendance and progress of the children in the family.
·Makes frequent reports to supervisor on progress of client services; maintains statistical records and performs other record keeping functions as required by agency.
·Recommends and obtains supervisor approval of limited financial assistance for client needs.
·Organizes time and manages tasks so that maximum service is given to clients and job performance meets agency standards.
·Maintains accurate, complete and timely client records as required by agency policy and procedures.
REQUIREMENTS
Education /Job Experience/Certification :
·BSW or Bachelor’s Degree in Human Services or related field·Two (2) years of case management experience·Experience working in either child development, public housing systems or family systemsSpecial Knowledge and Qualifications:
·Excellent written and verbal communication skills
·Ability to work independently and as part of a team
·Ability to provide culturally competent services
·Able to maintain a flexible schedule
·Must possess vehicle and current, valid driver’s license and insurance
