SOCIAL WORKERS NEEDED IN FLORIDA, HOME BASED!!!

To provide intensive social work support in identified cases of members who meet enrollment criteria, focusing on members who are high and/or moderate risk. This includes a variety of support to members with serious or severe physical illnesses, developmental disabilities, experiencing EOL issues, caregiver burnout or complex psych/social barriers that interfere with obtaining maximum benefits from qualifying programs.
REQUIREMENTS
Education/Training

-Social Worker (BS, or MSW) from an accredited school
-3+ years experience in clinical social work
-Current unrestricted license in the state that they reside.
-Certified Social Worker in Health Care (C-SWHC) preferred.
-Current member of the National Association of Social Workers preferred

Specialized Knowledge/Skills:

-Windows environment (preferably Word and Excel)
-Excellent written and verbal communication skills.
-Excellent organizational skills.
-Current PPD.
-Current valid driver’s license and required level of automobile liability insurance coverage.
-Thorough knowledge of social work practice and the application to case management including complex case management and behavioral health.
-Understanding of family and group dynamics.
-Familiarity with change behavior techniques.
-Knowledge of government and private organizations, resources in the community, laws, regulations and policies that govern clinical social work.
-Skill in establishing rapport with members, families, and local resources (medical, social, civic, legal, religious organizations)
-Ability to communicate and execute work effectively.
-Preferably a member of the local community.

Working Conditions:
?Environment – Independent, home environment with daytime travel within an assigned specified catchment area. Must have access to high-speed internet connection (e.g. DSL, Cable) and be able to comply with Work at Home Agent Requirements.
Travel: Willingness and ability to travel locally, reliable mode of transportation required.
