Outreach and Engagement Manager
Lead a team of outreach and engagement staff whose primary goals are to:1) Improve linkages among DCF workers, treatment providers and community-based support systems.
2) Provide holistic services to maximize the likelihood of client success.
3) Decrease the rate of failure of client engaging in recommended evaluation and treatment services.
4) Ensure successful client treatment outcomes.
5) Ensure the client receives quality treatment services in an appropriate treatment setting. In order to achieve these goals the team provides outreach, client engagement, support, counseling, direct assistance, training, advocacy and linkage services.Responsible for all clinical and administrative supervision related to all cases assigned to outreach workers in the region. Responsible to work with Case Management Liaison, to oversee and monitor outreach and engagement activity of all client cases. Responsible for providing outreach and engagement coverage and for small caseload of active cases. Primary contact with regional behavioral health providers, regional DCF representatives, and regional DMHAS teams. Additional responsibility includes clinical utilization management of outpatient services and coordination of responsibilities with Project SAFE program manager. Position is based in Middletown (33%) with the balance of time in each of the two regions served (66%).Direct Supervision of Outreach and Engagement staff
1) Provides individual supervision to all outreach and engagement professionals.
2) Provides weekly or biweekly group supervision to all outreach and engagement professionals.
3) Provides orientation, on going training, and feedback to staff.
4) Reviews outreach and engagement professional caseloads and activities related to caseloads and make recommendations related to quality and effectiveness of interventions.
5) Insure that all interventions are appropriately documented in the electronic case record.
6) Work with staff to identify and manage clients with high-risk behaviors.
7) Assesses and addresses areas needing improvement.
8) Follows agency standards of progressive discipline.
9) Completes staff performance evaluations per agency guidelines.
10) Fosters professional development of staff by promoting education and training’s.
11) Review and approve all outreach worker requests for client discharge or transfer.Direct Intensive Case Management Functions
12) Responsible for providing coverage for outreach and engagement professionals.
13) Responsible for directly providing full array of outreach and engagement professional services, including but not limited to;
All case management services that assist clients in obtaining basic supports needed to improve their likelihood of success in recovery. (see outreach worker job description).
All care management services that guide the course of client treatment through various levels of care (see outreach worker job description).1) Services may include transporting clients in personal vehicles.
2) Documentation of interventions and activities related to client care.Administrative Duties
1) Participate in planning and development of the service delivery system and implement identified program changes.
2) Support the overall functioning of the Project SAFE program.
3) Provide clinical utilization management.
4) Participate in hiring process of new personal.
5) Liaison with community providers and social service agencies to develop and maintain collaborative relationships.
6) Liaison with Regional DSS offices to develop and maintain collaborative relationship.
7) Liaison with host agency staff to develop and maintain collaborative relationship.
8) Monitor staff activity and productivity and make decisions related to utilization and impact of services.
9) Keep program manager informed of all significant client or program related issues.
10) Complete reports as requested.
11) Review all critical incidents.
12) Attend agency sponsored or recommended in-services, outside workshops, conferences and meetings.
13) Perform other assignments and duties as required.
REQUIREMENTS
Possess mastery of the principles and practices involved in delivering outreach based case management services to a behavioral health population with primary disorders related to substance abuse.
Masters required, license desired.
Must have at least four or five years of related experience.
Demonstrated ability to supervise outreach workers for effective and efficient service delivery.
Demonstrated leadership, organization and planning skills.
Cultural competency.
Excellent verbal and written communication skills.
Must have valid driver’s license and reliable transportation.
Must be willing to provide community-based services with significant travel.
Must be willing to respond to pager during regular work hours and potentially provide after-hours pager coverage.
Must possess proficient PC skills - (Windows NT and Outlook 98)
