The Johns Hopkins Hospital is seeking an On-Call Social Worker.

Responsibilities:Provides direct psychological, social and practical care to a select caseload of patients and families including social work assessments problem identification, goal setting and implementation of care plansMakes referrals to outside agencies and coordinates continuity of careWorks collaboratively with the multi-disciplinary team to optimize patient outcomesContributes to the attainment of organizational goals
Social Worker will be on- call 11pm-8am for one week every three weeks and is required to provide coverage during absences
