Nurse Practitioner - Colorado Springs (Set your own schedule!)

We're looking for a talented Nurse Practitioner to join us in this autonomous role. We offer orientation to long-term care, including preceptors and ongoing support. Make a direct impact in the lives of the people we serve and enjoy the freedom to set your own schedule when you apply your expertise with us.



Primary responsibilities include:


Making on-site visits to long-term care facilities to provide care to aging residents


Managing chronic conditions as necessary


Working actively and collaboratively with physicians to manage a caseload of 80-100 patients on a monthly basis


Building relationships and maintaining positive communication


Setting up appointments and managing a caseload


Making rounds with physicians
Participating in care conferences with patients and families

Job Requirements:

The successful candidate will be a licensed NP. National certification in family, adult, or geriatric is preferred. Acute care certification is also acceptable. Long-term care experience is a plus. RN experience is also required.
