Community Council for Mental Health and Mental Retardation, Inc. is seeking a Licensed Social Worker.
FUNCTION:
Provide a continuum of counseling support service that includes group, individual, family, and community components such as: psycho educational/support groups, parent/child sessions, risk assessments, supervised on-site visitation supports, management of crisis and referral to behavioral health system, direct counseling services, implement in accordance with service plan, and crisis management to families within the Achieving Reunification Center. Design behavioral strategies in collaboration with multidisciplinary team and family.
RESPONSIBILITIES:
Provide direct service in individual, family, and group counseling sessions.
Collaborate with the multidisciplinary team, educational team, and the family in implementing and coordinating service planning.
Complete individual care plans, and other clinical documentation to maintain clinical records in compliance with Agency policy and procedures and DHS requirements.
Develop material used for psycho educational/support groups that will provide structured learning opportunities for parents.
Provide weekly schedule of specialized psycho educational support groups to agency supervisor.
Schedule and Coordinate weekly appointments/sessions.
Conduct periodic visits in the home, school, and/or community according to care plan.
Ensure that groups will operate from a curriculum designed to identify and promote those skills most essential to successful reunification.
Provide parent/child sessions that give parents the opportunity to enact the skills they are learning in the groups.
Conduct risk assessments, referrals, and agency intake as needed and/or required.
Provide management and support of behavioral health crisis at the center.
Provide specialized on-site visitation support, utilizing filial play therapy, to ensure that the needs for positive bonding experiences are addressed, family members given the tools and the opportunities to develop these bonds.
Supervise the implementation of the care plan and evaluate to ensure continued progress.
Conduct family-centered case consultation.
Participate in staff meetings, in-service training, staff development and clinical case conferences.
Backup other clinical and supervisory staff as requested.
Perform other duties as requested.

Founded in 1967, Community Council provides comprehensive adult residential, mental health, outpatient, pre-school family intervention, partial hospitalization, psychiatric, psychological, school based, substance abuse, and Wraparound (BHRS) services. Our dedicated employees provide a large array of these services at the main facility in West Philadelphia, which sits on a beautiful five-acre campus. Employees also provide specialized programming in several Philadelphia public schools. Our residential services are provided in two locations within the West Philadelphia community.
As a base service unit, Community Council provides services primarily in West Philadelphia. However given the rapid and exciting growth within the organization, we have expanded services to cover several regions within the Philadelphia area.

REQUIREMENTS
EDUCATION, TRAINING AND EXPERIENCE:
LSW/Master’s degree in a human services discipline.
Experience in working families with children/adolescents with behavioral and emotional problems.
Ability to complete clinical and administrative documentation as required by agency and DHS standards.
