LPN - Licensed Practical Nurse Full time
This evening position is 3:00 pm - 11:30 pm with Tuesday and Wednesdays off.
Would you like to work in a home-like environment with developmentally disabled adults? This position is both rewarding and challenging. LPN's provide instruction in quality nursing care for residents by the Resident Assistants and assists in direct resident care as appropriate. Assesses medical needs of residents on unit and notifies RN/physician as deemed appropriate.
Responsibilities:
1. Performs treatments and procedures as ordered by the physician with the understanding of its cause and expected effects. Assists, observes and provides instruction to Residential Aides in all aspects of personal (feeding, bathing, changing) and/or nursing care (treatments, medications, g-tube care) for consumers.
2.Assists with the scheduling and documentation of all nursing related appointments, lab work, exams and consults. Communicates appointment schedule to supervisors and day programs, as needed. Assists in taking individuals to appointments, including the provision of safe transportation to and from appointments.
3.Observes symptoms, reactions, lab results and health needs of each consumer. Accurately records (progress notes) and reports information to the RN and Physician as deemed appropriate.
4.Obtains and documents monthly (or more often as directed or needed) vital signs, weights and breast exams, as applicable.
5.Implements appropriate therapy programs for each consumer, as directed by the Plan of Nursing Services, including but not limited to anodyne therapy, BART, Therapeutic Feeding, AFO and splint application.
6.Transcribes, signs and administers prescribed medications for each consumer as needed. Updates the Medication Administration Records as needed; checks medication deliveries for accuracy and count, monitors and reports medication errors to the RN. Maintains appropriate medications and medical supplies as needed.
7.Assists the RN in establishing and maintaining Physician Orders, DNR orders and medication histories for each consumer. Assists in the writing of goals, monthly notes, quarterly notes and annual reports of each consumer as directed by the RN.
8.Communicates pertinent health information (dietary changes, medication changes, potential for side effects, etc) through shift reports, progress notes and day programs. Communicates daily with RN (or RN on Call) for any unusual incidents or health care concerns.
9.Attends and participates in consumers’ program planning meetings as requested.
REQUIREMENTS
Qualifications:
1.Must be currently registered as a NYS Licensed Professional Nurse.
2.Physical ability to be on feet all day and perform lifting/positioning of consumers as required by responsibilities listed below.
3.Must possess a valid driver’s license and driving record acceptable to the agency insurance carrier.
4.Prefer at least one year experience as a LPN.
5.Knowledge of OMRDD regulations preferred.
