GAP Case Manager

The Gilead Apartment Program (GAP) Case Manager is responsible for following through with the daily operations of a 24- hour, seven day-a-week support program servicing adults recovering from serious and prolonged mental illness and co-occurring disorders. An active member of the client’s treatment team, the GAP Case Manager is responsible for providing a full array of case management services, which includes but is not limited to clinical assessment, reporting and documentation, as well as rehabilitation- and recovery-oriented treatment. Communicates professionally and cooperatively.Documents all case management activities within required time frame and in required format – including documentation of all client related activities into electronic database, submitting data and reports as defined and within required time limits and maintaining up to date and accurate hard copy of clients’ records with appropriate documentation and signatures, which include quarterly treatment plans in accordance with the Quality Assurance guidelines. Consistently communicates professionally and cooperatively.Maintains internal communications – notifies supervisors with all relevant client or program-based information. Attends and participates in all required staff, supervision and training meetings. Complies and works towards goals established by DMHAS and Gilead. Adheres to all State and Federal Confidentiality Regulation, HIPAA Regulations and must maintain confidentiality of all HIPAA related policies and procedures.Models positive behavior relative to following policy and procedure and can be relied upon relative to attendance and punctuality.
REQUIREMENTS
Education: BA Degree in Human Services required
Experience and skills: Minimum 1 - 2 years exp in community mental health preferred.
