Family Care Coordinator

Our Family Care Coordinators are dedicated professionals who are committed to the well being of the community and the children they serve.

Some of the position responsibilities are:

. Maintains established caseload productivity requirements; maintains standards for contact and interaction.

. Maintains client confidentiality according to established policies, federal & state standards for obtaining needed releases of information.

. Provides complete documentation for services rendered within established time frames, daily service logs, progress notes, assessment and case plans.

. Completes assigned paperwork in compliance with standards, including time sheets, leave requests, client records, client appointments and the management information system.

. Provides quality services to clients and families.
REQUIREMENTS
Bachelor's Degree required; preferred in Social Work, Psychology or related human service field; one year's experience working in social service area preferred.
