Extended Hours Coordinator
POSITION SUMMARY:
The Extended Hours Coordinator reports to the Director of Clinical Operations. Responsibilities include providing direction and supervision to all Extended Hours and PRN Nursing staff and to ensure that all patient care needs are met after regular business hours.
GENERAL PERFORMANCE:

6.Technical: Achieves and maintains appropriate level of performance with respect to agency technology: computer-based communication, data management, and report development.
7.General: Complies with annual employment and education requirements. Adheres to dress policy and is neatly groomed.
FUNCTIONS:
1. Provides direction and supervision to salaried, On-call, Weekend RN and PRN staff.
2. Monitors and assists, as necessary, the referral/admission process during weekend hours and ensures that assignments are made to meet the patient/family needs.
3. Is competent to input referral/admission information into the electronic medical record.
4. Demonstrates competency in performing clinical assessment.
5. Is able to communicate effectively with physician, family members and referral sources.
6. Is able to revise current and/or develop new processes that will better meet the needs of our patients/families after regular business hours and develop policy for these processes as appropriate.
7. Facilitates the observation and evaluation process of all Extended Hours and PRN staff.
8. Demonstrates understanding of State and Federal regulations, JCAHO standards and Hospice of Northeastern Illinois policies and procedures.
9. Attends a minimum of ten (10) in-house in-services per year.
10. Elevates level of knowledge and competence by assuming responsibility for personal development.
11. Actively supports the agency and its mission by serving as an advocate on end-of-life issues in the
community and participating in agency events.
12.Participates in clinical administration on-call rotation.
13.Service to patients/families.
RESOURCE MANAGEMENT
1.Assists with team budget and assigned resources based on census. Understands and considers relationships between clinical operations and resource consumption in decision-making.
2.Assists in development and maintains processes to monitor, evaluate, and adjust staff levels to deliver patient care and support in accordance with agency standards for high quality care.
COMPLIANCE
1.Maintains current knowledge of and compliance with all federal (i.e. Conditions of Participation, HIPAA) and state (i.e. IDPH Licensure, IL Nurse Practice Act) regulations and voluntary (i.e. JCAHO) accreditation standards applicable to provision of agency services (i.e. eligibility criteria, documentation requirements)
2.Assures all services are provided in accordance with agency policies and procedures and clinical standards. Actively participates in development, review and revision of agency policies and procedures and clinical standards.
3.Actively participates in formal and informal Performance Improvement activities (i.e. clinical record review) and agency committees, as assigned.
REQUIREMENTS
QUALIFICATIONS:
1.Previous experience in hospice, home health, oncology, geriatrics or general medical/surgical nursing.
2.Demonstrated commitment to hospice philosophy of care.
3.Has available means of transportation that allows for reliable visits.
4.Knowledge and continuing awareness of technology associated with pain management and palliative care.
5.Experience using email, the Internet, Microsoft Office products and other software (i.e. electronic medical record) as appropriate.
6.Volunteer training within one year of employment preferred.
EDUCATION:
1.Registered nurse currently licensed in the state of Illinois.
