• Work with children and families identified by the Local Systems of Care as needing assistance in the development of a community based care plan and in making and effectively utilizing linkages with community providers.• Provide Level III Care Coordination services for those children and families with a child meeting SED criteria that are in need of independent care coordination services.CHC was formed in 1972 by a group of volunteers who were passionate about helping low-income families and individuals. As result of that commitment, CHC now treats thousands of patients from over 100 communities in its 22 locations. REQUIREMENTS
Qualifications:
• BA/BS in Human Services area, or Social Work, or Psychology
• Exp working with adults or children with severe emotional or behavioral difficulties
• Exp working with community agencies, clients and families, community based care providers
• Multicultural awareness and ability to work effectively with people of varying cultural backgrounds
• Prior care coordination/case management experience preferred
• Valid Connecticut driver’s license
• Access to automobile for community based assessments and meetings
