Southwest Behavioral Health Services is one of Arizona's leading behavioral health agencies, with over 40 locations in the Phoenix area and in Payson.

This position provides screening, assessment, treatment planning, case management, crisis intervention, individual and/or group counseling services to recently discharged, seriously mentally ill adults in an in-home or semi-supervised residential setting. Functions as an integral part of an interdisciplinary team to develop and implement treatment or case plans, assessment criteria and programmatic goals, objectives and activities. Provides clinical assessment. Develops treatment plans for clients in conjunction with team members. Conducts home visits as needed and case consultations with other agencies and organizations. Monitors caseload for re-authorizations.

REQUIREMENTS
Required to be available weekend, evenings, nights and holidays as scheduled.
Minimum requirement consists of a 21-year age, Master’s degree in the Behavioral Health field (Psychology, Counseling, or Social Work) plus two (2) years of full-time behavioral health experience & Motor Vehicle clearance. Good academic and working knowledge of DSM IV, State SMI check list, Community Resources, Psychotropic Medications, Crisis Intervention, Clinical Methods and Procedures, Intervention Techniques and ability to apply same to possible unstable, treatment resistant adult/adolescent/child of various ethnic cultures for co-occurring disorders (GMH/SA/SMI). Knowledge and ability to function as a team, compile, organize and document client assessments, intervention techniques, treatments, case planning and follow-up. Possess excellent verbal and written communication skills, provide quality internal and external customer service skills to effectively dialogue with all contract and funding sources.
