Casemanager (Transitional Living Program)

Position Summary:
The Case Manager assesses the social service needs of assigned clients and provides direct casework assistance to clients within agency policies, procedures and practices.

Essential Duties and Responsibilities:
•Carries a caseload under supervision, providing direct individual and family services with emphasis on meeting the psycho-social needs of clients.
•Completes a comprehensive assessment or reassessment of assigned clients within the guidelines established by the agency.
•Obtains and evaluates information necessary to implement client services.
•Develops a written service plan for each assigned case within required timeframes; implements client service plans; monitors service plan progress; and frequently reviews progress with supervisor.
•Maintains client face-to-face contact through required number of home visits each month.
•Assesses and monitors the safety of clients in current living arrangement.
•Assesses permanency options; recommends permanency plan for clients; and implements plan to achieve permanency for clients.
•Studies, learns and uses agency procedures for providing services to clients. Explains agency’s services to clients and families and assists them in the utilization of all available resources.
•Maintains contact with relevant family members, assesses their need for services, and works on relationship building between client and family.
•Contacts courts and other social service agencies or community organizations to coordinate client services or to make inter-agency referrals.
•Develops a working knowledge of community resources to use in serving clients and maintains productive working relationship with these resources.
•Makes frequent reports to supervisor on progress of client services; maintains statistical records and performs other record keeping functions as required by agency.
•Provides linkage to other appropriate resources within specified referral procedures; secures all written agreements in accordance with guidelines established by contracting agencies and other outside sources.
•Recommends and obtains supervisor approval of limited financial assistance for client needs.
•Organizes time and manages tasks so that maximum service is given to clients and job performance meets agency standards.
•Maintains accurate, complete and timely client records as required by agency policy and procedures.
•Reports client progress in court proceedings through clear written reports submitted as required, and through accurate verbal testimony.
•Attends staff meetings, required training sessions inside and outside the agency, as well as other professional workshops to enhance job skills.
•Performs other related tasks as required or assigned.
REQUIREMENTS
Education /Job Experience/Certification:
•BSW or BS/BA in Human Service field.
•Licensed Child Welfare Services Employee or license eligible.
•0-2 years experience working in human services, child welfare, or crisis intervention.

Special Knowledge and Qualifications:
•Effective verbal and written communication skills.
•Ability to work both independently and a part of a high functioning team.
•Car and valid Illinois driver’s license required.
