Case Manager - YOUTH

Steininger Behavioral Care Services is a private, not for profit organization providing Southern New Jersey communities with a comprehensive range of rehabilitative mental health services. We are currently seeking a motivated individual to join our Youth Case Mgt. team as a Case Manager.

As a member of the team, the case manager assists youth with serious emotional disturbance or court ordered youth and their families to connect with needed services and stabilize the child’s home environment.

Responsibilities:
·Clinical assessment
·Charting-generates accurate and timely documentation of service provision, summaries of client progress, etc.
·Discharge planning
·Outreach
·Psycho-education
·Referral/linkage
·Service planning
·Supportive counseling
·Transports clients to appointments and other activities efficiently and safely

REQUIREMENTS
Qualifications:
·BA in a health related field
·MA preferred
·2 years POST BA experience in child welfare, counseling & advocacy/negotiations
·Current driver’s license in good standing
