Case Manager/Social Worker

JOB DESCRIPTION - Work with customers under the supervision of the Program Manager, and with other staff to meet customer and Support Services Program objectives.
JOB TASKS - Case managementl; counselng; customer intake, basic needs assessment; develop and implement Individual Service Strategy; record-keeping; community awareness and education; and. on-going assessment and monitoring of customers, service referral and activity follow-up to insure quality of service delivery.
EDUCATION/SKILLS REQUIRED
BA/BS in Social Work, Psychology or related field.
Applied knowledge of principles of case management, counseling and general assessments.
Ability to express ideas clearly and concisely in writing or orally.
Ability to maintain accurate records and prepare correspondence and reports.
Basic knowledge of Windows based office productivity software (Word, Excel, Access, etc.,)
