Case Manager Aide
The SCOH (Services to Children in their Own Homes) department is currently seeking a Case Manager Aide to support the Case Manager with non-direct services such as:
*Identify structured interventions that promote the development of life skills such as:
-Parenting skills, family planning, source management, meal planning, and preventive health
*Identify community resources that address the Family Service Plan such as:
-housing, education and special education, vocational training and rehabilitation, health, mental health, drug and alcohol services, legal services, employment, and childcare
*Attend conferences on behalf of the case manager or in collaboration with consumers which includes:
-school,report cards, IEP's, discipline meetings, provider meetings, joint meetings with key personnel, discharge planning (residential, hospitalization, court), medical follow up (mental health, D/A, dentist, child link, BHRS), shopping for clients, and resource building
NHS is an EOE.
REQUIREMENTS
*High School diploma or GED required
*1-2 years of experience working within the human services field
