Case Manager

Works in a team environment to provide linkage and referral to mental health consumers, within our county, to needed and appropriate services. Completes assessments, develops treatment plans, advocates and assists consumers to become connected to appropriate services in the community. Within in a "team" structure provides monitoring to insure successful re-integration into the community.
REQUIREMENTS
Master's degree preferred, Bachelor's degree required from an accredited college or university. Bachelor’s degree candidates must have at least 2 years of related experience. A medical related AA degree in direct service plus 2 years related experience. Or 4 years of related experience working with mental health consumers.
Knowledge of community resources and agencies. Valid driver’s license. Willingness to work a forty (40). Willingness to accept on-call rotation. Willingness to work as a Team member
