Thresholds' Woodstock, IL location is seeknig a Case Manager to provide case management services to Thresholds members in a recovery-based model. This person will promote the mental, physical, emotional, and social well-being of our members by helping to instill independent living skills and fostering a therapeutic environment through the use of problem solving, counseling, and relationship-building techniques. The Case Manager will provide support though advocacy and access community resources; coordinate with collateral agencies, funding sources, and families. The Case Manager will perform comprehensive assessments, and help develop and implement treatment planning. This person also facilitates groups and documents interventions on a timely basis.

Qualifications Required:

- Minimum of BA/BS in Social Services or related field.
- Must have a Driver's license and car insurance
