Bachelors Degree Preferred
1 to 2 years related experience required
Responsibilities: To provide quality, cost effective management and coordination of care for complex acute admissions. To develop, implement, and manage outcomes of clinical care maps. Obtain clinical data and or Medicare guidelines in order to determine appropriate level of care, collaborate with payer representatives, coordinate health care services throughout the hospital stay. Maintain client confidentiality and provide service with privacy and patient rights of the individual as a priority.Qualifications: Current AZ. RN license. Prior experience with Case Management is preferred. Two to three years of clinical experience is required.
