About the Opportunity
Serve as advocate for the patient and family regarding delivery of quality care throughout the period of hospitalization and management of financial resources. Manage the relationship with payers and referral sources, assist in the discharge planning process and ensure financial reimbursement of each case. Able to make appropriate health care decisions regarding patient's capacity to make decisions, use of a health care surrogate, guardianship and power of attorney.

Responsibilities:


Assure that patients meet long-term acute care criteria from admission through discharge and that patient care is delivered in the most cost-effective manner
Perform assessment and intervention of patient's psychosocial and spiritual needs
Work with the interdisciplinary team and administration to monitor patient/ family satisfaction; respond to questions and complaints from patient, family members and payers regarding the care provided
Discuss appropriateness of a continued stay in long-term acute care with the multidisciplinary team, third-party case managers, physicians and other departments; maintain positive rapport with referral sources, referring physicians and discharge planners
Identify quality improvement, risk management indicators and risk occurrences on chart review
Monitor the financial resources of the patient and assist in defining and implementing financial strategies
Discharge plan: evaluate the patient's/family's wishes and resources to develop a discharge plan; communicate with patient, family and staff to re-evaluate the plan; use appropriate referral resources or other agencies to implement the plan Home: initiate educational programs to provide a safe transition to home; Refer patient to home health and DME agencies Skilled: update M.D. of patient's discharge plan and condition, provide community resources that may be available to patient and offer grievance counseling as needed; provide alternative placements: make referrals, provide clinical data and complete pre-admission screening



Qualifications


Requires RN or Licensed Social Worker with clinical health care experience in a rehab or acute care setting, preferably with Case Management Certification.
Should have experience as a Case Manager in insurance, workman's compensation or medical management.
Must possess the expertise to provide written reports that assure approval of continued stay and maximum reimbursement at discharge.
