Disease Manager Registered Nurse

This position will be responsible for the provision of disease management program activities for outpatient and inpatient settings, in the civilian and direct care system, in support of TriWest’s overall disease management program. Activities are coordinated with facilities’ multi-disciplinary team members with focus toward a disease/demand management strategy.
