Clinical Transformation Director - Moore Grant Project

The 47-bed Novato Community Hospital is a not-for-profit, general acute care hospital accredited by the Joint Commission on Accreditation of Healthcare Organizations. Serving its community since 1961, the hospital affiliated with Sutter Health in 1985. Novato Community Hospital works in close partnership with Marin General Hospital to provide for the healthcare needs of more than 64,000 North Marin County residents. Novato Community Hospital recently opened a $40 million replacement hospital and a medical office building on the Novato Medical Campus.

 Featuring a $38 million replacement hospital, which opened in early 2001, Novato Community Hospital is a 46-bed acute care facility serving more than 64,000 residents of North Marin County. The hospital's services include a 24-hour emergency department, an ambulatory care center, diagnostic imaging, surgery center, cardiac care and laboratory services. Novato Community Hospital patients have access to additional services, such as obstetrical care and cardiac catheterization laboratory services, at nearby Marin General Hospital through a collaborative arrangement between the two facilities.

 

This position is responsible for overseeing and implementing the activities related to the Moore Foundation Grant Project of  “Transforming Nursing to Provide Excellence in Patient Care and Patient Safety Outcomes”, which is focused on improving Patient Safety practices and outcomes, and implementation of Evidence Based Practices (EBP) at Marin General Hospital (MGH) and Novato Community Hospital (NCH).  Oversees the MGH and NCH process of identifying, implementing and evaluating the performance of Evidence-based practices.  Provides training for the MGH and NCH council members and staff.  Provide leadership and act as a change agent to facilitate and support the development of the facility council.  Coordinates the activities of the Facility Council as related to PS and EBP activities.  Provides Report on progress related to target goals to the Facility Executives Team and Quality Committees.  REQUIREMENTS
Licensure:

Active license as California Registered Nurse required. 

 

Education: 

Bachelor of Science in Nursing or clinically related field is required.  Masters in Nursing, Business, Public Health or related field preferred.  Certified Professional in Healthcare Quality (CPHQ) preferred. Specific training in quality improvement principles and methodology, as well as project or healthcare related management is preferred.

 

Experience:

Demonstrated clinical competence as may be gained through 5 years in clinical nursing experience.  Demonstrated project and people management. 

 

Knowledge:

Basic knowledge of patient safety and evidence-based practice literature.  Knowledge relating to healthcare delivery, project management, and quality improvement is preferred. Demonstrated ability in leading or facilitating teams is required. Proficiency in Microsoft Word is required and preferred for Excel, Access and Outlook. 

 

Skills: Patient Safety and error prevention concepts and principles. Quality improvement techniques and principles. Basic clinical skills, knowledge and background. Facilitation and project management. Organizational skills. Ability to communicate effectively with a wide range of departments, and individuals at all levels within the organization. Ability to function independently and effectively in a dynamic environment, develop timelines and meet deadlines. Creative, self-motivated, and the ability to plan and initiate new activities.
