Case Manager, Psych

Org Marketing Statement Independently/interdependently monitors and coordinates the progress of a particular patient
population and improved/cost effective patient care delivery.
Minimum Education: Licensure in the State of Colorado as a mental health professional Master’s degree required. Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Social Work - (LCSW), Nursing - MSN
Minimum Experience: 2 years of experience as a case manager or 2 years. Experience in a psychiatric inpatient or outpatient program with demonstrated clinical competence.
Testing Required. Other Information Psychiatric Case Manager - Home Based Clinician:
This is a full-time (M-F) position for a licensed (LMFT, LPC, LCSW) mental health professional based in the UCH Outpatient Psychiatric Clinic. Primary responsibility is to provide home-based psychological and psychosocial services to youth and their families. Duties include case management, individual and family psychotherapy, assessments and psycho-educational interventions for clients, significant others and stakeholders. The ideal candidate will have outstanding clinical, communication and collaborative skills for working in a multidisciplinary environment. This community based position requires that one have personal transportation throughout the greater metro area. Knowledge of MST a plus.

Conditions of EmploymentAs an Employee you will need to be able to exerting 50 to 100 pounds of force occasionally and/or 25 to 50 pounds of force frequently and/or 10 to 20 pounds of force constantly to move objects.
