RN Care Manager

Responsibilities Include: Assessing, planning, implementing, coordinating, monitoring, and evaluating short-term and acute care management services within the company’s Health Management Programs.

Job Requirements


REQUIRED: WA-licensed RN (CCM, BSN preferred). Must have at least 2-years experience in Care Management, Medical Case Management, Disease Management, or Population Management in Asthma, Diabetes, or Children with Special Healthcare Needs. Must be self-directed professional with knowledge of community resources, current practices and procedures, acceptable medical treatment and diagnoses. Excellent organizational, interpersonal and communication skills are key as well as proficiency with PC based computers and ability to learn info systems and software.

PREFERRED: Experience with ambulatory nursing with strong asthma, diabetes and pediatrics. Background in discharge planning and home health plus knowledge of state, federal, and third party regulations and standards (CHAMPUS, L&I, SSI).
