ICM Care Planner

Department: MSSP/ICM/IAH Purpose: The care manager helps the clients by assessing their situations and then refers or obtains required services to maintain their independence in their own homes.Responsibilities:
Conducts in-home assessments to determine eligibility of a client for the ICM Program. Ensure client’s needs are met through written care plan approved by the Program Manager and discussed verbally with the client and/or his representative. Ensure that the client’s needs are met on an ongoing basis by contacting the client via a telephone call a minimum of one time per month, face to face contact a minimum of every quarter and an annual assessment. Ensures appropriate and timely communication of information about cases and ensures historical record of case-related activity by creating and recording case documentation, care plans, service plans, service authorizations that are accurate, timely and complete. Ensures effective arrangement and coordination of services for clients and effective working relationships with providers by communicating and interacting in a timely, cooperative and professional manner. Ensures and maintains County purchase of service dollars as well as maintaining purchase of service dollars donated from the United Way for client needs, track and document all expenditures. Candidates must have strong organizational, written and verbal skills. Candidates must be team players and computer literate. Experience with home visits, crisis intervention and Bilingual Spanish required.

Job Requirements



Education:
A Master’s degree in social work, psychology, counseling, rehabilitation, gerontology or sociology plus one year working with the elderly; or a Bachelor’s degree in one of the above fields plus two years experience working with the elderly is required. Candidates must have strong organizational, written and verbal skills. Candidates must be team players and computer literate. Experience with home visits, crisis intervention and Bilingual Spanish required.
