We're looking for caring, flexible RNs to train in our Houston office and work from one of our dynamic TX locations. Primary responsibilities will include:
Conducting in-home visits for high acuity patients (Medicaid members)
Working with physicians and family members
Arranging care
Developing treatment plans
Monitoring care
Assisting members in navigating through the care system
Participating in conference calls and attending office meetings as needed


Job Requirements:

Current RN licensure in Texas is required, along with a clinical background (preferably in Med/Surg, ICU, Home Health - Medicare/Medicaid). Must be self-directed, extremely reliable and organized. Bilingual fluency in English/Spanish or English/Vietnamese is preferred. Case Management experience is also desired.
