As a LPN/ RN Licensed Nurse you will:



• This position will perform the observation and data collection function for all adult clients and derive nursing diagnoses based on observation and data collection data. Act as a patient advocate. Implement nursing interventions to maximize health status and minimize symptoms associated with acute or chronic disease. Deliver care within the scope of nursing practice and in accordance with accepted standards of practice. Practice universal precautions in all contacts with patients, and special isolation techniques as warranted by patient diagnosis. Teach and demonstrate care to be provided by the patient, family, or home health aide. Provide ongoing supervision and teaching as warranted. Provide emotional support to patients and families in a manner that fosters independence, affirms existing and new strengths, and promotes and develops strengths. Respect the autonomy and right to confidentiality of all patients and families. Identify need for intervention by other members of the interdisciplinary team on an ongoing basis. Evaluate the effectiveness of care in conjunction with other members of the interdisciplinary team. Report changes in patient condition to the Clinical Manager. Document care provided and the response of the patient/family; submits documentation in a timely fashion as described in agency policy. Provide for continuity of service across care settings through effective information management, including both verbal and written communication.


Qualifications

•Acceptance of Philosophy and goals of this agency

•Six months experience in area of specialty/desired practice

•Current unrestricted licensure to practice as a LPN, RNin the state(s) to which assigned and current CPR certification

•Good organizational and communication skills

•Ability to function with minimal supervision

Position needed

Part time and Full time positions Available:
1. South East Michigan Region Visiting Nurse. (Macomb, Wayne, Oakland, Lapeer, Genesee and Washtenaw Counties)
2. Work on our Managed Care Team for High Risk Patients In the Field.
3. Bowel and Bladder Program for lifetime patients In the field.


Contingent Rates will be the following:

Visit rate: $28.00 to $35.00 depending on experience for LPN

RN Rates:

SOC: $85

Visit: $50

Recert: $75

Resumption of Care: $75

Transfer: $5

Salaried LPN or RN: Negotiable
