Registered Nurse CAPD Exemption Status: Non-exempt
Summary:
The Registered Nurse for home dialysis (CAPD) is responsible for providing education training and support for the home dialysis patient.
Qualifications:
1) Current RN license in the state of practice.
2) One year of experience in dialysis.
3) Training/teaching experience in the education of patient, family or staff.
4) Current CPR certification.
5) One or more years of dialysis experience preferred.
Essential Job Functions:
Candidate Selection:
1) Evaluation of Peritoneal Dialysis candidates.
a) Counsel pre-dialysis patients in the choices of treatment modalities.
b) Counsel patients during change modalities.
c) Work closely with nephrologist and social worker in evaluating patient for program.
2) Ability to evaluate home environment in order to meet treatment specifications.
3) Works closely with nephrologist and social worker to screen patients for program.
Patient Care:
1) Develop and maintain nursing care plans.
2) Prepare patients to assume primary responsibility for own health maintenance.
3) Orient patients to the peritoneal dialysis program.
4) Train the patient in peritoneal techniques.
5) Provide written resources pertaining to peritoneal dialysis.
6) Monitor, order, and mail supplies as needed for the home dialysis patient.
7) Coordinate care when home dialysis patient is hospitalized.
8) Perform tubing changes for CAPD/CCPD patients.
9) Provide emergency care as needed.
10) Make periodic visits to the home for surveillance of performance and adaptation.
11) Maintain records. Evaluate patient monthly in clinic, i.e. labs, blood pressure, weight adequacy, and ongoing patient education.
Quality Assurance / Financial Management:
1) Charges for patient care services according to DaVtia policy.
2) Complete flow sheets and charge vouchers.
3) Order intial home dialysis supplies. Monitor usage.
4) Participate in quality assurance cost containment program.
Communication and Patient Service Skills:
1) Ability to articulate instruction for patient health improvement, problem solving, or a mechancial problem when speaking with patients over the phone.
2) Through knowledge of ESRD, CAPD/CCPD and hemodialysis patient training.
3) Must provide detailed, and accurate documentation of all patient communications, and health status changes in medical file.
4) Maintain contact with patient physician or nurse practitioner.
5) Follow orders, protocol, and procedures as assigned by either the physician, or the immediate supervisor.
6) Administer patient quality care in a considerate, respectful manner.
Other Duties as Assigned:
1) Attend staff meetings, CQI meetings, and educational activities.
2) Compliance with DAVITA policy and procedures in accordance with state and federal regulations relating to patient care and services. (Medicare, OSHA, or other governing agencies)
3) Compliance with state/federal regulations of the Injury Prevention and Safety Training Program.
4) Overtime hours may be required, with little or no notice, in order to meet the needs of the unit.
5) Ability to perform all range of motion; walking, sitting, bending, reaching, stooping as required for performing dialysis therapy.
6) Ability to lift objects weighing up to 35 lbs.
7) Understand and follow DaVita patient and employment policy and procedure.
