EDUCATION AND LICENSE
Masters Degree from an accredited school of social work
LCSW or LCSW-C State of Maryland Licensure required.
Provide telephonic case management services to the geriatric population as referred to Social Work team by MHS clinicians. Completes psychosocial assessments to identify and apply services and resources to address, social, health or economic needs of the beneficiary. Works closely with the MHS clinicians to ensure the provision of holistic care to facilitate best health outcomes. Monitors the effectiveness of psychosocial interventions, and provides consistent feedback to the MHS team on progress. Works with community based groups to address service gaps and serves as a liaison to social, health, and psychiatric resources.RESPONSIBILITIES INCLUDEComplete bio-psychosocial telephonic assessments to develop personalized goals and interventions designed to meet the preferences and needs of the beneficiary and caregivers.Assist beneficiaries caregivers with difficult or complex care delivery and serve as advocate in matters related to procuring of resources and servicesUtilize knowledge of benefit plan design, eligibility and/or financing alternatives available to provide appropriate information, referrals, and coordination of services.Assist in the monitoring of provider based discharge planning from all levels of care in accordance with established criteria and makes recommendations for post-discharge needs to the team and/or provider where appropriate.Identify situations where vulnerable adults may be at risk for abuse and/or neglect and intervene for APS referrals, when indicated.Provide education and support to beneficiaries /caregivers around advance care planning DPOA, hospice.Develop strategies for managing beneficiaries/caregivers with significant behavior/psychiatric issues.Provide education and trainings to MHS clinicians around identifying psychosocial related issues for social work intervention.•Perform duties consistent with all State and Federal regulatory requirements, accreditation organization guidelines, health plan and Healthways guidelines.
