Description: These are exciting times for the residents and employees at the Cedar Hill Health Care Center. Right now, we’re in the process of opening a sub acute floor for our residents. Licensed Social Worker

Reports to: Director of Social Services

Job Description:

The primary purpose of this position is to assist in the planning, organizing, implementing, evaulation and directing the overall operation of the Social Service Dept. in accordance with current federal and state and local guidelines and regulations as well as in accordance with established policies and procedures. To ensure that the medically related emotional and social needs of the resident are met and maintained on an individual basis.


REPONSIBILITIES:

Develop resident care plan in areas of behavior, mood and psycho-social needs.
Participate in discharge planning, development and implementation of social care plans and resident assessments.
Collaborate with staff members, community agencies, consultants to resolve the needs and problems of the resident/family through the development of social service programs.
REQUIREMENTS
SW, Social Work, MSW preferred
Long-term care experience preferrerd
Mental health background desired
