Service Case Manager - Norristown Case Management

FT, salaried position. Responsible for a caseload of Spanish speaking adults, children and families with serious and persistent mental illness.
Will focus on the following consumer outcomes:
a.independence of living
b.vocational/educational participation
c.adequate social supports
d.accessing necessary services
e. reduced hospitalizations

Job activities will include the following:
a.assessment and understanding of the consumer’s history and present life situation.
b.service planning.
c. translating for individuals at Intake, medication checks, and psychiatric evaluations, as needed.
d. work with intake staff to coordinate appointments.
e. receive initial contact calls from Spanish speaking adults, children and families to determine course of service.
REQUIREMENTS
In addition to fluency in the Spanish language,
a Service Case Manager will have one of the following:
1. A Bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling or education.
2. Be a registered nurse
3. A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, and other social science and 2 years experience in direct contact with mental health consumers and fluency in the Spanish language.
