Responsibilities Include: Providing one on one patient care, in clients homes. Assessment and evaluation of client’s needs and implementation of established plan of care. Coordination of care with supervisors, families and other interdisciplinary team members. Documentation to meet state standards, providing accurate and timely document submission to facilitate patient care. REQUIREMENTS
Education: Graduate of a school of nursing Knowledge and Abilities: Knowledge and ability to effectively implement and document nursing theory, practice, and principles. Good communication and interaction skills. Experience: Prefer a minimum of one year nursing experience Special Requirements: Current licensure or permit by the state Board of Nursing, driver’s license, CPR certification, and automobile liability insurance
